Key Responsibilities

Provide physical therapy services to patients within a home health setting across the Skagit and North Snohomish coverage area
Conduct evaluations, develop individualized treatment plans, and deliver skilled PT interventions in the home environment
Document patient care accurately and in a timely manner using Homecare Homebase (HCHB)
Coordinate with the interdisciplinary care team to ensure continuity and quality of patient care

Required Qualifications

Active Physical Therapist (PT) license in the state of Washington
Homecare Homebase (HCHB) experience required
Prior experience in a home health setting
Valid driver's license and reliable transportation for home visits within the coverage area

Preferred Experience / Skills

Familiarity with the Skagit and North Snohomish County regions
Strong organizational and time-management skills for managing a home-based caseload
Experience working independently in a field-based therapy role
